How to find an in-home caregiver in Hilton Head Island

Hilton Head Island is an incorporated town in Beaufort County, and the Census Bureau's estimate for July 2025 was 38,196 people. According to the ACS 2020–2024 survey the median age is 59.8, and 14,855 residents (39.2%, margin ±2.2 points) are 65 or older, including 2,217 who have reached 85. More than half of the island's 18,326 households (53.0%) contain someone 65-plus. In 3,289 of them an older person lives alone, 2,238 of those women. About 27% of residents over 65 report a disability, and 6.3% of households have no vehicle.

Those numbers shape what a good post says. Spell out whether your parent still drives, which tasks matter most (getting up and dressed, meals, a shower with someone nearby, medication reminders, company in the afternoon), and the days and hours. Spanish is the home language of 12.7% of island residents aged five and up; note it if your father would rather be cared for in Spanish. Ask each applicant where they would be traveling from. The profiles listed above are sorted nearest first, reach up to 50 miles, and change as people join. The neighboring Bluffton page covers the mainland side of the county, about 8 straight-line miles from the island's ZIP centers.

Evaluating caregivers in Hilton Head Island

No check of any kind is done by CaregiverConnect, so the screening is yours. If an applicant says they are a certified nurse aide, confirm it with the South Carolina Nurse Aide Registry; SCDHHS warns that holding a card does not prove current certification. Ask for two references you can phone, and ask them about punctuality and how the person coped when things went wrong. Interview in the home where the work will happen, then book a short paid trial. On an island the storm plan is part of the job: ask how the caregiver would reach you before an evacuation and whether they could return afterwards, because the Town says residents re-entering after an evacuation need a photo ID showing a local address or a utility bill with their name and island address.

Questions families here ask at interview

  • From your home, how long does the weekday-morning drive onto the island take you?
  • If an evacuation is ordered, could you help me get my mother packed with her medications, and would you be able to come back once re-entry opens?
  • Our house has steps at the entry; have you helped a walker or wheelchair user manage stairs safely?
  • Would you drive Dad to Hilton Head Medical Center visits, and in whose car?
  • How would you keep an afternoon interesting for a parent who lives alone and sees few people?
  • Could you go over a new prescription with my mother in Spanish?
  • If my parent qualifies for Community Choices later, would you be willing to enroll as a self-directed attendant?
Schedules and cost

What in-home care costs in Hilton Head Island: hourly rates and schedules

Island families often ask for an early visit covering the shower and breakfast, several afternoons a week of company and errands, and daily help for a short stretch after a hospital stay. Families who live elsewhere often want a set check-in time and a text afterwards. If you need many hours, plan on more than one caregiver: when Medicaid pays for attendant care, SCDHHS asks that every effort be made to keep each attendant at 40 hours a week or less.

Who is the employer. When you hire a caregiver directly, you are the employer. The Federal (South Carolina has no state minimum wage law) minimum wage is $7.25 an hour (effective July 24, 2009); the federal minimum of $7.25 and federal overtime rules apply to directly employed caregivers unless the work is exempt companionship under the FLSA. BLS groups Beaufort and Jasper counties into one metro area, so the island has no separate figure. In the May 2025 wage data the middle aide wage there was $14.85 an hour; a quarter of aides earned $14.48 or less and a quarter $16.87 or more, with the top tenth starting at $17.36. That is above the South Carolina median of $14.48 and below the national $17.21. Nursing assistants in the metro had an $18.14 median. Agencies cost more: CareScout's 2025 survey puts the South Carolina agency rate for non-medical help at a median of $31 an hour. The state never passed a minimum wage law, and the U.S. Labor Department says FLSA-covered employers in South Carolina owe at least the federal $7.25. Independent caregivers set their own rates, so settle pay for travel time and any driving before the first shift.

No agency tracks private rates. A caregiver you hire directly will usually expect more than the employed-aide median and less than an agency rate; every caregiver on this site lists an hourly rate on their profile, and the hiring guide covers taxes and overtime.

Safety

Hiring safely in Hilton Head Island

Agree on paper the hourly rate, schedule, duties, mileage and notice period, and keep weekly timesheets signed by both of you. Keep jewelry, checkbooks and extra prescriptions locked, and ask the bank for account alerts. Federal wage law has covered domestic service workers hired by private households since 1974, but companions for people who cannot care for themselves are exempt from its minimum wage and overtime rules, and live-in workers are exempt from overtime only. A July 2, 2025 Labor Department proposal would restore the 1975 version of these exemptions; its direct care page shows which rules apply now. For 2026, IRS Topic 756 sets $3,000 in cash wages to a single household employee as the point where Social Security and Medicare withholding generally begins. When you suspect that a vulnerable adult is being harmed, neglected or exploited, South Carolina DSS Adult Protective Services takes reports by phone, 1-888-227-3487 (1-888-CARE4US), and online; dial 911 if someone is in danger. Questions

Hiring a caregiver in Hilton Head Island: frequently asked questions

Can South Carolina Medicaid pay for a caregiver at home on Hilton Head Island?

Possibly, through the Community Choices waiver, an SCDHHS alternative to nursing home residence. It covers frail people 65 and over, and adults aged 18 to 64 with a physical disability, if they meet the nursing facility level-of-care criteria. Island residents start with the Community Long Term Care Beaufort line, (843) 521-9191, or the statewide intake number, (888) 971-1637.

Could I, as the daughter or son, be the paid attendant?

Often yes: SCDHHS lets a self-directed attendant be a relative, subject to the state's Family Caregiver Policy. Barred from pay are the parents, stepparents and foster parents of minor participants, and anyone legally responsible for spending their own assets on the participant's care. Before enrolling, an attendant must show a tuberculosis blood test or skin test result, and must give references if the participant asks.

Who supervises a self-directed attendant?

The participant or a representative directs the work, sets the visit times and chooses the tasks. SCDHHS says a licensed nurse certifies that this supervision is safe, and extra training can be provided when the nurse judges it necessary or someone asks for it. Travel time to and from the home is not counted in the authorized hours.

How much do private caregivers charge on the island?

Rates are set by each caregiver and appear on their profile. For comparison, payroll aides across the Beaufort-Jasper metro earned a $14.85 hourly median in May 2025, while agencies statewide charged a $31 median for non-medical help in CareScout's 2025 figures.

Where can a family caregiver on the island get support?

The Lowcountry Area Agency on Aging has a Family Caregiver Advocate at 843-473-3973 for information, education and services, and its I-CARE counselors at 843-473-3968 explain Medicare, Medicare Advantage, supplement and drug plans, and Medicaid.
