How to find a caregiver in East County San Diego
Two hiring markets share this page. In La Mesa, Lemon Grove, El Cajon, Santee, Spring Valley and Lakeside, 10 to 21 road miles from downtown, caregivers live close by (median home values of $634,400 in Lakeside and $681,700 in El Cajon against $854,700 county-wide), the Orange, Green and Copper trolley lines bring caregivers without cars to homes near the stations, and postings draw applicants quickly. From Alpine outwards (Pine Valley, Descanso, Campo, Julian, Borrego Springs) the pool thins to nothing: no caregivers have joined from Julian or Borrego Springs yet, and a posting there depends on someone in Alpine, Lakeside or El Cajon willing to drive 30 to 75 miles. The cards above come from a 40-mile radius around El Cajon.
Languages matter in specific places. El Cajon has the county's largest Arabic-speaking community (11.0% of residents aged five and over), so caregivers who speak Arabic with an older parent exist here in a way they do not elsewhere; Spanish-speaking caregivers are easy to find for Lemon Grove, Spring Valley and La Presa. State the language in the posting and interview in it.

Types of in-home care families in East County San Diego arrange
All non-medical. For nursing tasks (injections, wound care, catheters) you need a licensed home health agency, not a private caregiver.
- Senior care at home: meals, reminders, housekeeping, errands, company.
- Personal care: bathing, dressing, grooming, toileting, transfers.
- Companion care: conversation, walks, appointments, errands.
- Dementia care: routine-based support; ask for experience.
- Respite care: shifts so a family caregiver can rest or work.
- Overnight and live-in: night supervision or room-and-board arrangements.
- Post-surgery recovery: non-medical help after a hospital stay.
- Part-time and flexible: a few hours on set days is the usual start.
Evaluating caregivers in East County San Diego
Check what can be checked. An IHSS provider number means the Public Authority has taken the caregiver's SOC 426, run a Live Scan fingerprint check and had them sign the provider agreement; the county's El Cajon IHSS office on North Magnolia Avenue handles the recipient side. A Home Care Aide Registry listing is compulsory for the aides of the region's 28 licensed agencies (13 in La Mesa, nine in El Cajon) and voluntary for independents. A CNA certificate from CDPH is most often earned locally at the Grossmont Health Occupations Center in Santee, and experience at Sharp Grossmont, Grossmont Gardens, Covenant Living at Mount Miguel or one of the region's 53 six-bed care homes can be confirmed with a reference call. Then interview with your relative present, phone two previous families, order a consumer-reporting background check with written consent and book paid trial shifts. CaregiverConnect does not run checks; a Verified badge reflects proof a caregiver uploaded.
Questions families here ask at interview
- Which trolley line or freeway exit are you near, and how long is the drive to us at 8 am?
- Are you enrolled with IHSS, on the Home Care Aide Registry, or a CNA from the Grossmont Health Occupations Center?
- Which languages do you speak with the person you care for: Arabic, Spanish, English?
- Would you drive my mother to Sharp Grossmont, and how do you charge for the time?
- Have you cared for someone with dementia, and what do you do when they refuse care or wander?
- Have you worked at Grossmont Gardens or Covenant Living, and why did you move to private work?
- How far up I-8 will you travel, and would you take a full-day shift in Alpine or Pine Valley?
- What is your rate, and what is extra for overnights, weekends and driving?
Typical schedules and what care costs in East County San Diego
Suburbs and backcountry post differently. In the trolley suburbs the usual posting is three to five weekday blocks of personal care and company, with dementia supervision common in Lakeside and Alpine (19.7% and 19.5% of residents aged 65 or older) and overnight cover after a discharge from Sharp Grossmont. El Cajon families often ask for an Arabic-speaking caregiver for a grandparent in a multigenerational household. From Alpine outwards, families post full days rather than split shifts, because a caregiver will not drive 14 miles from El Cajon for a two-hour visit, and the oldest communities in the county, Julian (43.9% aged 65 or older) and Borrego Springs (40.6%), may need someone to stay several days at a time.
| Reference point | Hourly | Source |
|---|---|---|
| Employed aides, San Diego metro (San Diego County) | $16.97 median | BLS OEWS, May 2025 |
| IHSS provider wage, San Diego County | $20.40 | CDSS; effective March 1, 2026 |
| California minimum wage | $16.90 | effective January 1, 2026 |
| Agency non-medical home care, California median (CareScout 2025) | $40.00 | CareScout Cost of Care Survey 2025 |
Who is the employer. When you hire a caregiver directly, you are a household employer under California's Domestic Worker Bill of Rights. The state minimum wage ($16.90 an hour from 1 January 2026; some cities set more, for example $17.75 in the City of San Diego) is the legal floor, and a personal attendant earns overtime at 1.5× after 9 hours in a day or 45 in a week. The main exceptions are IHSS recipients and an employer's own parent, spouse or child. Source: California DIR, Domestic Worker Bill of Rights FAQ.
Rates in the East County suburbs sit between the $20.40 IHSS wage and the $40 agency median with a small travel premium because caregivers live close; the backcountry reverses that, and a Julian or Campo posting should carry mileage or a day rate. No East County city has its own minimum wage, so $16.90 applies, not the City of San Diego's $17.75. Median household income is lower in El Cajon ($67,511) and Lemon Grove ($88,009) than in Santee ($113,394) or Alpine ($108,660), so IHSS is the first thing to check: apply through the county's El Cajon office or the AIS line, and if your relative is 65 or older, blind or disabled and on full-scope Medi-Cal, the county pays the caregiver you choose for the authorised hours. Privately paid hours carry household-employer taxes and overtime after nine hours a day.
No agency tracks private rates. A caregiver you hire directly will usually expect more than the employed-aide median and less than an agency rate; every caregiver on this site lists an hourly rate on their profile, and the hiring guide covers taxes and overtime.

Hiring a caregiver in East County San Diego: frequently asked questions
Can I find an Arabic-speaking caregiver in El Cajon?
More easily than anywhere else in the county: 11.0% of El Cajon residents speak Arabic at home. Put the language in the posting, say whether the client also speaks English, and interview in the language your parent uses.
How quickly will an East County posting get applicants?
In La Mesa, El Cajon, Santee, Lemon Grove and Spring Valley, usually within days, because many caregivers on the platform live there. Alpine and Lakeside take longer; the backcountry has no local caregivers yet.
Where do I apply for IHSS in East County?
Phone Aging & Independence Services at (800) 339-4661 or visit the county IHSS office at 389 North Magnolia Avenue, El Cajon. Hospital discharge planners can also refer online.
What should I pay a private caregiver in La Mesa or El Cajon?
Between the $20.40 IHSS wage and the $40 agency median, with little travel premium in the suburbs. Add mileage or a day rate for Alpine and beyond.
Is there anyone to hire in Julian or Borrego Springs?
Not yet on the platform. Families there can still post; a wide posting radius and a day rate with mileage give a caregiver in Alpine, Lakeside or Ramona a reason to take it.
