IHSS San Diego: pay rate, how to apply, and how to become a provider

In-Home Supportive Services (IHSS) pays a provider to help a Medi-Cal recipient stay at home, and in San Diego County it is run by Aging & Independence Services with a separate IHSS Public Authority in National City handling provider enrollment and the caregiver registry. This guide covers IHSS San Diego from both sides: how a family applies, what a provider is paid, how enrollment works, and which number to call. Every figure comes from a county, Public Authority or state page checked on 14 October 2026.

What IHSS is and who qualifies in San Diego County

The state describes IHSS as in-home help for "eligible aged, blind, and disabled individuals" so they can remain safely at home instead of moving to a facility. The county lists the help as bathing and grooming, dressing, cleaning the house, shopping, laundry and making meals.

To qualify in San Diego County you need to:

  • meet the financial eligibility guidelines (people on SSI/SSP qualify financially; others may qualify but pay part of the cost);
  • be blind, disabled, or 65 or older;
  • meet full-scope Medi-Cal requirements;
  • be unable to live at home safely without help, and live in your own home (CDSS does not count a hospital, nursing facility or licensed care home as your own home).

The recipient, not the county, is the provider's employer: CDSS makes the recipient responsible for hiring, training, supervising and, if needed, dismissing the provider. The State issues the paycheques.

IHSS San Diego pay rate in 2026

San Diego County pays individual IHSS providers $20.40 an hour, effective 1 March 2026, according to the CDSS county wage workbook in force from 1 October 2026. Counties set their own rates, so this figure applies only to San Diego County cases.

Reference wageHourlyDifference from IHSS
IHSS provider wage, San Diego County$20.40-
City of San Diego minimum wage (from 1 Jan 2026)$17.75$2.65 below
California minimum wage (from 1 Jan 2026)$16.90$3.50 below

Overtime. The provider agreement (SOC 846) sets the IHSS rules: time and a half after 40 hours in a workweek that runs from Sunday at midnight to Saturday at 11:59 pm. At the San Diego wage that works out at $30.60 an hour (illustrative calculation: $20.40 × 1.5). A provider working for more than one recipient may not claim more than 66 hours in a workweek, and travel time between recipients is capped at seven hours a week. All of it, overtime included, comes out of the recipient's monthly authorised hours, so your IHSS income depends on the hours the social worker approved, not on how many you are willing to work.

One local extra: Back-Up Provider System (BUPS) caregivers, who cover urgent short-term needs or hospital discharges, are paid an additional $2.00 an hour, per the Public Authority. The City of San Diego minimum wage covers work done inside city limits only; elsewhere in the county the state rate is the floor. The IHSS pay by county guide compares San Diego with other counties.

How to apply for IHSS in San Diego County

You can apply for yourself, or someone can apply for you. For a child under 18, apply using the child's information. The county accepts the IHSS application (form SOC 295) several ways:

  • Phone: call the Aging & Independence Services call center on (800) 339-4661 and apply with staff.
  • Email: send the SOC 295 to AIS.295only.HHSA@sdcounty.ca.gov (only the application; other forms go to your social worker).
  • Fax: (619) 344-8077.
  • Mail or in person: at any of the six IHSS offices listed below, which can also print an application for you.
  • Online referral by professionals such as hospital discharge planners, through the county HHSA web portal.

After you apply, an IHSS worker visits you at home to discuss your medical condition, living arrangements and the help you already get from family and friends, and how often you need each service. A health care professional must complete a certification of your condition and need for services; CDSS calls this the Health Care Certification (SOC 873) and says the county must receive it before services can be authorised. The worker then sends a Notice of Action saying whether you qualify, which services and how many hours, and whether you have a share of cost.

How long it takes. Neither the county IHSS page nor the Public Authority publishes a standard approval time, so treat any figure you hear as an estimate and ask the social worker where your case stands.

How to become an IHSS provider in San Diego

Provider enrollment is handled by the IHSS Public Authority, not the county call center. Its Caregiver Enrollment Department lists these steps:

  1. Request an enrollment packet by calling (866) 351-7722 or using the Public Authority's online request form. Have your recipient's case number ready.
  2. Attend an in-person appointment (the booking link comes in the packet). Bring the original SOC 426 enrollment form with every box filled and an email address; your Social Security card (original, unaltered); an unexpired government photo ID whose full name matches the card; a work permit or work authorization if required; the original SOC 426A designation form completed by the recipient; and the Request for Live Scan Service form with $47.00 in cash, money order or cashier's check payable to "Kingdom Security".
  3. Complete an online video session on Zoom (audio and video required); the link is sent after the first two steps.
  4. Sign the SOC 846 provider enrollment agreement electronically through AdobeSign, sent to the email on your SOC 426.

Missing any step delays enrollment. Once you are paid, timesheets go through the state Electronic Services Portal (etimesheets.ihss.ca.gov) in two pay periods a month, the 1st to the 15th and the 16th to month end. Direct deposit is available, and the Public Authority says timesheets take 2 to 3 calendar days to process.

Paid sick leave. For the 2026/27 fiscal year providers earn 40 hours from 1 July 2026, after first working 100 hours; you can use them after a further 200 hours or 60 calendar days, whichever comes first, and unused hours expire on 30 June. Claim them on SOC 2302 or in the portal.

The union. The Public Authority names United Domestic Workers of America (UDW) as the union representing IHSS caregivers in San Diego County, The Public Authority itself was set up under state mandate as the employer of record for IHSS providers and runs the registry; the County Board of Supervisors is its governing body. The state-level background check rules are explained in our guide to becoming an IHSS provider.

The IHSS Public Authority San Diego registry

The Public Authority also keeps a caregiver registry: recipients who already qualify for IHSS ask for a customised list of caregivers who match their schedule, language and approved tasks. No experience is needed to join. If you want registry referrals, call (866) 351-7722 before asking for an enrollment packet. The steps:

  • Call or submit the online inquiry form; staff reply within three business days.
  • Complete the registry application packet sent by email, by its deadline.
  • Attend a registry workshop on Zoom and pass a basic test on caregiving skills, the Public Authority and the IHSS program.
  • Pass the Department of Justice Live Scan check, and complete provider enrollment if you are not already an active provider.
  • Receive a welcome letter; the registry then starts sending your name to recipients.

For recipients, registry lists are processed within one to three business days. The registry also offers free online training courses to all IHSS caregivers.

IHSS and private care on CaregiverConnect

IHSS hours are capped by the recipient's assessment, and many families need more cover than the county authorises: evenings, weekends, overnight or respite for a relative who is the IHSS provider. Those extra hours are a private arrangement. On CaregiverConnect a family posts the need for free, caregivers in the area apply directly, and the family hires directly and becomes the household employer for those private hours, at a rate agreed between them (at least the state or city minimum wage). There is no commission. CaregiverConnect does not run background checks; caregivers may upload proof of one. Optional premium plans exist.

For providers, the same logic runs the other way: an IHSS case rarely fills a full week, so many providers add private clients. A free caregiver profile lets families in your part of the county find you. The California in-home care cost guide sets private rates against agency prices, and the family caregiver pay guide covers being paid to look after a relative.

Frequently asked questions

How much does IHSS pay in San Diego?

$20.40 an hour for individual providers, effective 1 March 2026 (CDSS county wage workbook). Overtime is paid at time and a half after 40 hours in a Sunday-to-Saturday workweek, within the recipient's authorised hours.

What is the IHSS phone number in San Diego County?

To apply for IHSS, call the county call center on (800) 339-4661. Providers enrolling, joining the registry or asking about pay should call the IHSS Public Authority on (866) 351-7722. Portal, direct deposit and sick leave questions go to the state IHSS Service Desk on (866) 376-7066.

Where is the IHSS office in San Diego?

The county lists six IHSS offices: El Cajon, Escondido, National City, Oceanside, Overland Avenue in San Diego and the Southeastern Live Well Center on Market Street. Applications are mainly taken by phone. The Public Authority, which enrolls providers, is at 401 Mile of Cars Way, Ste. 200, National City.

Can a family member be my IHSS provider in San Diego?

Yes. CDSS says providers may include family members, friends and neighbours. The recipient names the provider on form SOC 426A, and the relative then completes the Public Authority enrollment steps and the Live Scan check like any other provider.

How long does IHSS approval take in San Diego County?

The county does not publish a standard approval time. The steps are the application, a home visit by an IHSS worker, the Health Care Certification (SOC 873), which must arrive before services are authorised, and then a Notice of Action.

How do I join the IHSS registry in San Diego?

Call the Public Authority on (866) 351-7722 or submit its online inquiry form, complete the emailed application packet, attend the Zoom registry workshop and pass its basic test, and clear the Live Scan check. No caregiving experience is required.

Is IHSS income taxable?

Federal and state income tax is withheld only if you file a W-4 or DE 4 (SOC 846). A provider who lives with the recipient can exclude IHSS wages from federal and state income tax by filing SOC 2298 under IRS Notice 2014-7; this does not change Social Security (FICA) or Medicare. CDSS cannot give tax advice, so check with a tax preparer.
