IHSS Santa Cruz County: how to apply, Public Authority contacts and the $20.90 wage

In-Home Supportive Services (IHSS) in Santa Cruz County is run by the Adult & Long Term Care division of the county Human Services Department, while a separate IHSS Public Authority enrolls providers and keeps the caregiver registry. This guide covers the IHSS Santa Cruz County phone numbers, the two walk-in offices, the forms you will meet, how a caregiver gets enrolled and the $20.90 provider wage. Everything was checked against county and California Department of Social Services (CDSS) pages on October 15, 2026.

Who qualifies for IHSS in Santa Cruz County

IHSS helps older, blind and disabled people stay safely in their own homes when they cannot fully look after themselves or keep up with routine household tasks; the county describes it as an alternative to institutional care. Paid help falls into four groups: domestic and related work (meals, clean-up, laundry, grocery shopping, errands), personal care (moving around, transfers, bathing, repositioning and paramedical services), going along to medical appointments, and protective supervision for someone with a cognitive impairment.

The county's basic eligibility list:

  • 65 or older, blind, or disabled, with the disability expected to last at least 12 months (disabled children may also qualify);
  • living in Santa Cruz County, in your own home or a place you choose (the county does not treat an acute hospital, a long-term care facility or a licensed community care home as your own home);
  • able to stay safely at home with in-home help, and having functional limits that make self-care hard;
  • a completed Health Care Certification, form SOC 873, from a licensed health care provider;
  • meeting low-income rules, for example Medi-Cal with or without a share of cost, or SSI/SSP.

Santa Cruz County IHSS phone number and ways to apply

For a referral, the Santa Cruz County IHSS phone number is (831) 454-4101, or (831) 763-8800, option 2. You can also send the state application form, SOC 295, in any of these ways:

  • email to ALTC_Support_Staff@santacruzcountyca.gov;
  • fax to (831) 763-8906;
  • mail to IHSS Intake, P.O. Box 1320, Santa Cruz, CA 95061;
  • walk in at 500 Westridge Drive, Watsonville, CA 95076 or 1400 Emeline Avenue, 3rd Floor, Santa Cruz, CA 95060.

Once a referral is made it is confidential, so the county will not give status updates to anyone calling on someone else's behalf.

What happens next. IHSS confirms Medi-Cal eligibility and provides a Medi-Cal application if needed. A licensed health care professional completes the SOC 873. A social worker visits your home, reviews that visit along with information from your medical providers, and sends a notice: if approved, it lists the tasks and monthly hours; if denied, it gives the reasons.

The 45-day clock. You have 45 calendar days from the date the county asks for the SOC 873 to return it, and CDSS says no services are authorized before it arrives. Without it, or acceptable alternative documentation, the application is denied. If you have a substantial reason beyond your control, contact your social worker before the 35th day to ask for another 45 days. Instead of the SOC 873 you can send a licensed professional's signed statement, dated no more than 60 days before you submit it, describing the condition and which daily activities you cannot do alone. If approved, eligibility may start from the application date.

Hiring your IHSS provider

The person receiving IHSS is the employer and hires, supervises and lets go of the provider. CDSS says the same and adds that the employer trains and directs the work. The county notes that recipients can usually hire anyone, including a spouse, other relative, friend or neighbor, provided that person has the legal right to work in the U.S. and a valid Social Security number.

If you are hiring on your own, the county suggests a phone screen, then a face-to-face meeting, then reference checks, and telling the people you did not choose. Once you have decided:

  1. Call (831) 454-4101 for form SOC 426A, Recipient Designation of Provider, with your case number and the provider's full name or provider number ready. If the provider came through the registry, call the registry on (831) 454-4036 instead.
  2. Fill in every section and return it to IHSS, P.O. Box 1320, Santa Cruz, CA 95061, ATTN: IHSS/426A.
  3. Your provider should receive timesheets within 10 business days.

Never sign a blank timecard, and check that the hours claimed each day match the hours worked. If you have a share of cost, pay the provider the amount shown in the letters CDSS sends you. For interview questions, see hiring a private caregiver; they apply to IHSS hires as well.

IHSS pay in Santa Cruz County: $20.90 an hour

Santa Cruz County providers earn $20.90 an hour. That figure, in force since January 1, 2026, appears in the CDSS spreadsheet tab "IP Effective 10-1-2026" (which we downloaded October 15, 2026) and in the county's July 2026 provider recruitment flyer, which also promises full health benefits for those who qualify. Santa Cruz shares seventh place among the 58 counties with Napa.

Monterey$21.047/1/2026
Santa Clara$20.441/1/2026
San Benito$19.351/1/2026

For comparison, the California minimum wage set by DIR is $16.90 from January 1, 2026.

We checked the Labor Center at UC Berkeley, whose list of local wage ordinances does not include the City of Santa Cruz, Watsonville or any other Santa Cruz County locality, so for private work the state $16.90 is the floor. The IHSS rate is $4.00 above it (illustrative arithmetic). The Public Authority serves as employer of record for collective bargaining on wages and benefits, and posts the provider MOU on its page.

Limits and sick leave. Providers working for two or more recipients may not go over 66 hours a week combined, except in limited cases CDSS allows. Paid sick leave is 40 hours for the 2026/27 state year that opened on July 1, 2026; it is credited after your first 100 IHSS hours, unlocks after 200 more hours or 60 days, whichever arrives first, and anything unused disappears at the end of June. File for it with the paper SOC 2302 or online through the Electronic Services Portal (ESP). Rates for all 58 counties sit in our statewide IHSS wage table.

Enrolling as a provider with the Santa Cruz IHSS Public Authority

No one can be paid by IHSS until enrollment is complete and the background check has cleared. The Public Authority sets out three steps:

  1. Online. On the county's IHSS Independent Provider Enrollment Center site, enter your details, watch the required videos, sign the enrollment form electronically and book your In-Person Orientation.
  2. In-Person Orientation. Hand in your original Social Security card and an original, unexpired government photo ID with a signature, such as a California or out-of-state driver's license or ID card, a tribal ID or a U.S. passport. The names must match, and only originals are accepted. You will pick up the Live Scan form, meet SEIU 2015 representatives and attend a question session with Public Authority and IHSS Fiscal (payroll) staff. Late arrivals are turned away and must rebook.
  3. Live Scan. Get fingerprinted at a location of your choice; the state Attorney General's website lists them. There is a non-refundable fee. A previous background check for another job does not count.

When all three steps are confirmed and the Live Scan results are in, the Public Authority mails you a letter about your eligibility. Do not book the orientation until you have your original Social Security card, since you cannot enroll without it. You enroll only once, however many recipients you later work for; if more than a year has passed since your last IHSS paycheck, call (831) 454-4036 to ask whether you must enroll again. For the general process, see how to become an IHSS provider.

IHSS Public Authority registry in Santa Cruz County

The Board of Supervisors created the IHSS Public Authority in 2001 under state law and a local ordinance. It manages provider enrollment, runs the registry, matches recipients with screened providers, shares training information and helps both sides solve problems.

For recipients. You or your authorized representative can call (831) 454-4036 for a list of possible providers. A Public Authority social worker asks about location, language, schedule and care needs, then sends a matched list. The registry does not guarantee a match will work out, so the county advises interviewing and checking references yourself.

For providers. After enrolling, call (831) 454-4036 and ask for an IHSS Provider Registry Application to be mailed to you. Return it with at least two professional references. A social worker then interviews you in person, checks references and decides whether you meet the registry's requirements. The county says it is short of registry providers, especially those willing to work in North County (including Capitola and the City of Santa Cruz), in the San Lorenzo Valley and Santa Cruz Mountains, or with high-needs clients who use a G-tube or a trachea tube.

Supplies. The Public Authority has supplied gloves, face masks and hand sanitizer to providers. From July 2026, SEIU 2015 handles the monthly distribution at 18 Alexander St., Watsonville, and 517B Mission St., Santa Cruz; call its Member Action Center on 855-810-2015 for details.

Payroll forms and the IHSS Fiscal unit

IHSS Fiscal answers on (831) 454-7315 or at HSDIHSSFISCAL@santacruzcountyca.gov. Forms the county lists for providers and recipients:

  • SOC 829 to start, change or cancel direct deposit, with the SOC 831 fact sheet;
  • SOC 840 for a new address or phone number, mailed to IHSS Fiscal at P.O. Box 1320 in Santa Cruz;
  • SOC 846 Provider Enrollment Agreement;
  • SOC 2255 and SOC 2256 workweek and travel time agreements, and the TEMP 3000 overtime declaration.

Tax forms work differently. Do not download blank W-4 or DE-4 forms; call IHSS Fiscal, which mails pre-filled copies with a return envelope. When filled in, the provider mails the W-4 and DE-4 straight to the state's IHSS Payroll Management Unit (P.O. Box 1660, West Sacramento, CA 95691-6660); the county will not process them from its offices or drop boxes.

When you need more hours than IHSS approves

Authorized hours are a monthly cap. Families often need more: overnight help, weekend cover, or someone to fill in while the regular provider uses sick leave. Those extra hours are a private job between the family and the caregiver.

CaregiverConnect lets a family post that job at no cost; caregivers apply to it themselves, and the family interviews, hires and employs whoever it picks. No commission comes out of caregiver pay. The platform itself carries out no background screening; a caregiver is free to attach evidence of a check. Optional paid plans add some features. For what private home care tends to cost, see this state cost guide. Santa Cruz County providers with open time can join as a caregiver; families start here.
